I think every one has their own method, and sometimes it depends on the breed. I think smaller breeds are harder because it's more difficult to see them squat.
My dog is part boxer and was very easy to break. We started with taking him outside very often. We also started watching for signs that he had to go. He would prance around in small circles, smell the carpet, or squat. When ever we saw this activity, we would take him outside. We also gave him lots of treats for doing good!
At night and while we were away, we kept him in the utility room in a a big pet cage. This became his "safe place" very quickly, and after a couple weeks, he wouldn't go to the bathroom in it. As soon as we got up in the mornings or came home, we took him outside.
It's funny how "programmed" they get. I always do a lot of computer work at night. I would shut it off, fix my coffee for the next morning, and put the dog to bed. After a few weeks, when I shut the computer off, he started going to bed himself.
Good luck training him. Dogs truely are great friends.

It is alot easier when they are young, biggest thing is when the pup wakes up, shoot it outside because its not gonna be long. When it goes make it the biggest thing in the world in the dogs mind, alot of times it will almost go in the same spots because of the joy that we showed. I personally cut off water at around 6, shut the toilet bowl lids and all. air twice maybe 3 times before bed, start over the next day, try and get a pattern, dogs are creature of habit. Here's 3 ol wise sayings.... Dogs are creature of habit, youll get more out of attrition than pressure, and patience is a virtue. This will fall into all dogs all breeds.

I don't think your post is strange at all and I am glad your asking questions because some people just dive in and have no clue on training a pup and when it doesn't do what they want they apply to much pressure and they get aggravated and the dog gets unsure.
I agree with tailcrackin on this one.
I use a porter kennel and for the majority of the time the pup while inside stays in the kennel and is let out on a regular schedule as in when I get up in the morning and before I go to bed at night and during the day my wife lets the pup out quite often. I also cut the pups off at 6:00 pm when in the house.
I have had very good luck on my outdoor dogs by just keeping them outside for the most part and when I have let them in, I didn't make it long stays as in usually no more than 20 minutes or so and then returned them to the kennel and as time went on I would let them come in for more time gradually each week or so until they knew that the grass or kennel was to be used for their facilities.
